Why does my computer screen go black or display an "Out of Frequency" error message after changing in-game resolutions?

When selecting a resolution in the Video Options menu, the screen may go black and display an "Out of Frequency" error message or the game may display incorrectly. This occurs when you select a resolution that your monitor cannot display correctly.

If this occurs, you will need to exit the game, and then launch the game and create a new profile, which resets the options to default.
